Tabernacle Flowers or Sanctuary Lamp Donation form

If you or your family would like to be the designated donor for our Tabernacle Flowers ($35) or Sanctuary Lamp ($10) please complete this form and include it, along with your check to Our Lady of Humility or St. Patrick Church in an envelope marked “Tabernacle Flowers” or “Sanctuary Lamp.”

Deposit the sealed envelope in the tithing basket at Mass. The memorialized name will be printed in the bulletin for the week of your gift.  One family per flowers and lamp each week at Our Lady of Humility and St. Pat’s. No Tabernacle flowers are permitted during Advent and Lent.

Specific weeks cannot be guaranteed, but we will try our best.

Weekly Mass Intentions

If you would like to have someone prayed for in a special way, download and complete this form and place it in a sealed envelope. It can be placed in the tithing basket, mailed to the Parish Office, or dropped off at the Parish Office.  Mass intentions are $10.00 per intention.

If we cannot schedule your intentions at the time and date requested, we will schedule them as close to those days as possible.

Mass intentions can be weekdays (Monday, 8:00 AM at OLH, Tuesday, Wednesday, and Thursday, 8:30 AM at St. Pat’s, Friday, 8:00 AM at OLH) or weekends (Saturday, 4:00 PM at OLH or 6:00 PM at St. Pat’s, Sundays, 8:00/11:00 AM at St. Pat’s and 9:30 AM at OLH).

Please note: Intentions must be submitted two full weeks prior to the date you requested.
